It needs a plan of accountability laying out prescriptive authority and each side's responsibilities, plus clinical practice guidelines documenting the drug types and categories involved. It has to say when the collaborating physician is available if they aren't physically on site, what happens if every physician is unavailable, and how patients learn about that. The last piece is a mutual acknowledgment of both sides' obligations. The rule has the agreement signed and kept on site; nothing in it calls for sending the agreement to the board for approval.
