Community Health Worker. Remote, New Jersey

CVS Health is hiring a Community Health Worker in New Jersey. The job is full-time and remote, 40 hours a week. Pay runs from $88k to $190k per year. The schedule is Monday to Friday, 8am to 5pm EST.

What you'll do

• Lead community health worker pilot implementation
• Supervise community health worker staff
• Coordinate care with interdisciplinary teams
• Monitor program performance and quality metrics
• Ensure compliance with regulatory requirements

Requirements

• Three to five years care management experience
• Experience managing high-risk population programs
• Bachelor degree in health-related field
• Strong analytical and communication skills
• Experience leading community-based pilot programs (preferred)
• Familiarity with Medicaid and HEDIS metrics (preferred)
• Experience with diabetes management initiatives (preferred)
