Community Health Worker. On-site, Pennsylvania

CVS Health is hiring a Community Health Worker in Pennsylvania. The job is full-time at a clinic, 40 hours a week. Pay runs from $18.50 to $38.82 per hour.

What you'll do

• Connect patients to community resources
• Facilitate communication between care team parties
• Manage patient referrals and care plans

Requirements

• One year experience in social services
• Proficient computer skills
• Access to reliable transportation
• Ability to manage multiple priorities
• Community Health Worker certification (preferred)
• Associates or Bachelors degree (preferred)
• Experience with electronic medical records (preferred)
