Community Health Worker. On-site, Indiana

CVS Health is hiring a Community Health Worker in Indiana. The job is full-time at a clinic, 40 hours a week. Pay runs from $18.50 to $35.29 per hour.

What you'll do

• Connect patients to community resources
• Facilitate communication between care parties
• Manage patient referrals and care plans
• Provide culturally appropriate health education
• Document patient interactions in medical records

Requirements

• One year healthcare or social service experience
• Proficient computer skills
• Access to reliable transportation
• Ability to travel within community
• US work authorization
• Bilingual English and Spanish (preferred)
• Multidisciplinary team experience (preferred)
• Community Health Worker certification (preferred)
