The CQ modifier and what it changed
Claims have carried the CQ modifier since 2020 to show that an assistant furnished all or part of a service, and since 2022 it costs: Medicare pays 85% of what it would otherwise pay. It attaches when the assistant furnishes more than 10% of a service, with an exception for the final 15-minute unit of a treatment day where the therapist alone supplied at least 8 minutes. Providers not paid from the fee schedule, critical access hospitals among them, sit outside it. Elsewhere clinics build the schedule around it.

- Does a physical therapist have to be on site while a PTA treats?
Less often than it used to. Medicare moved to general supervision for assistants in outpatient private practice in 2025, meaning the therapist no longer has to be in the building, and 44 states already allowed general supervision of PTAs when CMS counted them. Where a state practice act is stricter, the state wins, so read the act where the job is.
- What does a home health physical therapist assistant job involve?
Driving between patients and treating them in their own front rooms, with the equipment you brought. Pay is often per visit instead of hourly, so documentation time and mileage decide what the visit is really worth, and the autonomy is higher because nobody else is in the house. - Can a PTA change a patient's plan of care?
No, though plenty inside the plan is yours. Inside the plan an assistant progresses an exercise, changes a parameter, stops something that's provoking symptoms and documents why. Anything that changes the plan itself, including re-evaluation, goes back to the physical therapist, and the plan gets rewritten on what you report.
