Community Health Worker. On-site, Illinois

CVS Health is hiring a Community Health Worker in Illinois. The job is full-time in patients' homes, 40 hours a week. Pay runs from $18.50 to $38.82 per hour. The schedule is Monday to Friday, 8:00am to 5:00pm, occasional weekends.

What you'll do

• Bridge members and healthcare providers
• Provide outreach and education
• Support socially and medically complex patients
• Communicate medical information to professionals
• Influence providers regarding patient preferences

Requirements

• Located in Champaign or surrounding area
• Travel within 90 mile radius
• Reliable transportation
• 1-3 years relevant experience
• Experience with Medicaid populations
• Community or outpatient setting experience (preferred)
• Mental Health First Aid training (preferred)
• Electronic Health Records experience (preferred)
